Ger\u00e7ekten uygulanmas\u0131 gereken durumlar olan ileri derecede sperm bozuklu\u011fu ve her iki t\u00fcp\u00fcnde t\u0131kal\u0131 olmas\u0131 d\u0131\u015f\u0131nda polikistik over sendromu olanlarda t\u00fcp bebek uygulanmas\u0131 do\u011fru de\u011fildir.<\/mark><\/strong><\/p>\n\n\n\n

Polikistik over sendromu olan ve \u00e7ocuk sahibi olmak isteyen kad\u0131nlara \u00e7e\u015fitli nedenlerle \u201dsenin \u00e7ocu\u011fun olmaz, sen k\u0131s\u0131rs\u0131n\u201d veya \u201dt\u00fcp bebek yap\u0131lmas\u0131ndan ba\u015fka \u00e7aren yok\u201d deniliyor. Halbuki, tam tersine polikistik over sendromu olan kad\u0131nlar\u0131n yumurtal\u0131k rezervleri ve \u00fcreme kapasiteleri teorik olarak di\u011fer kad\u0131nlardan daha fazlad\u0131r(1,2,3,4,5,6,7,8).<\/mark><\/strong><\/p>\n\n\n\n

1-<\/mark><\/strong>Polikistik over sendromu olan kad\u0131nlar\u0131n yar\u0131s\u0131ndan fazlas\u0131 gebeli\u011fi engelleyen ba\u015fka bir neden yoksa herhangi bir tedaviye ihtiya\u00e7 duymadan gebe kalabilmektedir(44). Hatta, gerek olmad\u0131\u011f\u0131 halde yap\u0131lan t\u00fcp bebek tedavisi ile gebe kalamayanlar\u0131n bir k\u0131sm\u0131 da kendili\u011finden bir s\u00fcre sonra gebe kalmaktad\u0131r(49,50,51,52,53,54).<\/p>\n\n\n\n

2-<\/mark><\/strong>Polikistik over sendromu olanlar\u0131n sadece bir k\u0131sm\u0131 yumurta geli\u015fimi ve yumurtlama olmamas\u0131 veya gecikmeli olmas\u0131 nedeniyle gebe kalmakta zorluk ya\u015famaktad\u0131r. (1,2,3,4,5,6,7,8,9).<\/p>\n\n\n\n

3-<\/mark><\/strong>Polikistik over sendromu nedeniyle gebe kalmakta zorluk ya\u015fayanlara t\u00fcp bebek tedavisi yap\u0131lmas\u0131na gerek yoktur(10,11,12,13,14,15,16,17,18,19,20,21,22).<\/p>\n\n\n\n

T\u00fcp bebek tedavisinin yenido\u011fan bebekte ve daha sonraki geli\u015fim d\u00f6nemlerinde otizm (autism) gibi yan etkilere neden oldu\u011fu son zamanlarda anla\u015f\u0131lm\u0131\u015ft\u0131r. Genlerin etkinli\u011finin (expression) de\u011fi\u015fmesi (DNA methylation) (epigenetics) sonucunda otizm ortaya \u00e7\u0131kmaktad\u0131r. (23,24,25,26,27,28,29,30,31,32,33,34,35,36,37,38,39,40,41,42,43).<\/mark><\/strong><\/p>\n\n\n\n

Bu nedenle ger\u00e7ekten uygulanmas\u0131 gereken durumlar olan ileri derecede sperm bozuklu\u011fu ve her iki t\u00fcp\u00fcnde (fallop t\u00fcpleri) t\u0131kal\u0131 olmas\u0131 nedenleri d\u0131\u015f\u0131nda polikistik over sendromu olanlarda t\u00fcp bebek uygulanmas\u0131 do\u011fru de\u011fildir. <\/p>\n\n\n\n

Halbuki, polikistik over sendromu nedeniyle gebe kalmakta zorluk ya\u015fayan kad\u0131nlar \u00e7ok daha zarars\u0131z, kolay uygulanan ve az masrafl\u0131 t\u0131bbi y\u00f6ntemlerle zaten \u00e7ocuk sahibi olabilmektedir. (10,11,12,13,14,15,16,17,18,19,20,21,22).<\/mark><\/strong><\/p>\n\n\n\n

Polikistik over sendromu nedeniyle \u00e7ocuk iste\u011fi tedavisine yeni ba\u015flayan yakla\u015f\u0131k 5 kad\u0131ndan birinin e\u015finin sperm say\u0131s\u0131nda ve hareketinde azl\u0131k veya \u015feklinde bozukluk g\u00f6r\u00fcl\u00fcrken, bu kad\u0131nlar\u0131n bir k\u0131sm\u0131n\u0131n da t\u00fcplerinden birinin ya da her ikisinin t\u0131kal\u0131 oldu\u011fu saptanmaktad\u0131r(45,46,47,48). <\/p>\n\n\n\n

Bu nedenle \u00e7ocuk iste\u011fi tedavisine ba\u015flamadan \u00f6nce polikistik over sendromu ile birlikte, sperm ile ilgili ve sperm ile yumurtan\u0131n birle\u015fti\u011fi yer olan t\u00fcplerin (fallop t\u00fcpleri) b\u00fct\u00fcnl\u00fc\u011f\u00fcn\u00fcn bozulmas\u0131ndan kaynaklanan sorunlar\u0131n olup olmad\u0131\u011f\u0131n\u0131n belirlenmesi gerekmektedir(45,46). <\/p>\n\n\n\n

Ancak, polikistik over sendromu\u2019na ek olarak sperm ve t\u00fcplerle ilgili sorunlar\u0131n da olmas\u0131 durumunda t\u00fcp bebek tedavisi d\u00fc\u015f\u00fcn\u00fclmelidir.<\/mark><\/strong><\/p>\n\n\n\n
